Bilingual -LMSW Case Manager Central Dallas Division
 
Description
The Case Manager is a qualified, licensed professional who is responsible for the development, supervision, management and evaluation of physical, occupational, speech therapists and social work services and the coordination of services within the agency to ensure the provision of quality patient care. Coordination of care, chart review, family interviews, and medication review.
 
Qualifications
    • Graduate of an accredited school program of social work with a Master's Degree.
    • Currently licensed to practice in his/her specialty in the state of Texas. LMSW or LCSW required
    • Knowledge base of the differences between physical, occupational and speech language therapy treatment approaches is recommended. Knowledge base of childhood development, diseases and disabilities.
    • One (1) year experience in a medical facility (hospital, clinic, rehabilitation center, etc.) Where the team approach to treatment is utilized. Relevant clinical experience, supervisory/management experience preferred.
    • Ability to adapt to a fast paced environment.
 
Duties
    • Communicates and documents conversations with doctors, nurses, therapists, community based agency case managers and office staff.
    • Acts as a referral source for the patient families provides information, researches and gives detailed feedback to families as needed.
    • Reviews patient care documentation, participates in patient care conference and coordinates clinical care between therapists and physicians involved in patient care.
    • Performs patient admissions in patient’s home.
    • Investigates and solves patient issues in a timely manner.
    • Provides direct patient services as needed and required, is available at all times during operating hours.
    • May supervise home health care administrative / support staff as required.
    • May participate in the development and implementation of clinical programs as needed.
    • Assesses and makes recommendations for patients needs based on service utilization and coordination of care review.
    • Performs all duties in compliance with agency policies and procedures. Adheres to all rules, regulations, code of ethics, guidelines and codes in federal, state and local laws which are concerned with the delivery of home based therapy services.
