Course Description:

Documentation is the written expression of the clinical reasoning and decision making process.  Start of Care and Re-Evaluation documentation are vital for:

  • providing the foundation for development of the Plan of Care and rationale behind that POC (dosage, interventions)
  • showing a clear picture of a child’s medically based therapeutic needs for the PCP and others involved in the child’s care
  • establishing a process through which payer sources can clearly see medical appropriateness of recommended services, as well as the impact of intervention being provided when applicable

Stakeholders such as clinicians, PCPs, as well as payer sources need to have a clear understanding of testing information and clinical observations, as well as how that information relates to the child/caregiver’s participation based goals within the home and community.  THERAPY 2000s SOC & Re-Evaluation templates will be reviewed and discussed in detail to provide guidelines as to how they might best be used to meet the varied needs of identified stakeholders.

About Our Speaker:

Kathy Clapsaddle, M.S., CCC-SLP

Speech Program Director

Kathy Clapsaddle, M.S., CCC-SLP is a Program Director for Speech Language Pathology for THERAPY 2000 and the Division Director for the Austin division.  She received her Master’s Degree in Speech Language Pathology from Texas Tech University Health Sciences Center in 1999.  Kathy considers herself to be a “therapy geek.”  She has served as a Speech Language Pathologist, a university instructor, professional educator, and a leader.  She is currently the Vice President of Research and Development for the Texas Speech Language Hearing Association, and she serves on the Editorial Board for Evidence Based Practices (EBP) Briefs.  Her areas of interest include service delivery, autism, emergent literacy development and augmentative and alternative communication. She has presented across the state on issues related to Speech Language Pathology services in school and medical settings.

Learner Objectives:

At the end of the training, participants will:

  1. Identify the purpose of Start of Care and ReEvaluation documentation.
  2. Describe what information should be included in the Assessment Report & Clinical Observations sections to meet the needs of different stakeholders.
  3. Discuss how the data gathered through the SOC and ReEvaluation process relates to development of the Plan of Care.
  4. Describe factors to consider when making clinical decisions regarding dosage.
